     Complete and submit this required Resale Certificate
     This Resale Certificate form is active so you can fill it out on-line and email it back.
     On the signature line please follow the promps to make your signature
     Digitally Encrypted.
     OUTSIDE USA:International companies submit business or applicable license.

2) If you have a website or photos of your retail shop provide the website address on the PreApplication below; IF NOT submit one trade related invoice showing purchases of $200+
3) Fill out the form below and click on 'SUBMIT APPLICATION'
Once your application has been approved, you will receive a Login ID and Password to view pricing and place orders.
